Notes must be recorded either during, or as soon as practicable after, every consultation, to ensure that health records are accurate and up-to-date at all times. Patient files stored electronically will remain on a password protected computer at all times, or may be stored on a secure server. Patient files must not be given to any person other than the practitioner unless the patient gives clear consent to provide their health information to another practitioner for purposes of referral or treatment in the practitioner’s absence. Patient health information is not to be transmitted electronically unless for personal research purposes of the practitioner, in which case the information is to be de-identified before transmission. If the practitioner wishes to share client information for professional development/research/supervision purposes, it must be de-identified and any potentially identifying case details removed from the information shared. Patient cases are not to be discussed in any public place for any reason.
Patient files are to be stored for seven years after the cessation of treatment. Files which document any serious adverse event, or any circumstance that is likely to be the subject of legal action of any kind must be kept indefinitely. All files for disposal shall be shredded before recycling.
In the event that a patient requires access to their health records, the patient may be given a photocopy of records, but must sign a statement that this copy has come into their possession, thus relieving the practitioner of responsibility for the security of the information given to the patient.
